Materials:

TRACE/Transition Curriculum Guide

This guide includes materials and data sheets for each transition domain which includes: vocational, independent living, recreation, Self-advocacy, community and adult education.. The purpose of the guide is that the materials be adapted to be an individualize process for each student.

Transition Domain Assessments- An easy to use assessment to determine the needs of students and develop IEP goals based on the students needs.

Advocacy Materials-Tools to help empower students and increase self determination.

Staff Training Logs –A monthly checklist of training tools for Para educators to train them on skills necessary for successful transition services for students.
