Data: Transition to Adult Living (Washington USD)

Assessment occurs at both the program level and individual student level. Individual student progress is closely monitored through bi weekly performance assessments. Each student is assessed on essential competencies necessary for independent living and employment. Each student has a portfolio with individual skill goals that are identified, systematically taught in school, community and work based settings, and then monitored for progress. All students have goals in the areas in daily living skills, employment skills and independent living and travel, however, the individual skills needed to achieve those goals vary according to student need.

The program is evaluated on the number and quality of student's exit or transition plan. When the student leaves the program, the following question must be addressed: Have adult living arrangements, employment and community access been established?

The program's effectiveness is determined by the quality of life it establishes for its participants. Because students vary in complexity of need, all post program outcomes will not be the same. However, the expectation is that all students will leave the program having participated in the development of their transition plan through real life activities that allow them to make informed choices.
